Are you an experienced Adult Social Worker with strong hospital discharge expertise, confident in managing complex cases and supporting the professional development of others?
We are recruiting a dedicated Senior Social Worker to join the Short Term Support Service (Central) operating across Preston and Chorley. Offering £35.00 per hour with immediate interview availability, this role is ideal for a skilled senior practitioner looking to balance complex clinical practice with supervisory and leadership responsibilities.
Key Responsibilities:
- Complex Case Management: Manage a reduced but highly complex caseload supporting adults across both community and acute hospital settings.
- Staff Supervision & Mentoring: Provide professional advice, supervision, and practice mentoring to Social Workers, Social Care Support Officers, and students.
- Management & Quality Support: Support the Team Manager with case progression, practice quality assurance, and day-to-day service delivery.
- Safeguarding & Timely Interventions: Drive statutory safeguarding enquiries, manage legal processes, and lead multi-agency working to promote independence and reduce long-term care needs.
What We Are Looking For:
- Qualifications & SWE: Qualified Social Worker with active registration with Social Work England (SWE).
- Post-Qualifying Experience: Significant post-qualifying experience in statutory Adult Social Care, demonstrating strong leadership behaviours.
- Essential Requirement: Proven Hospital Discharge experience is essential for this role.
- Skills: Confidence in managing risk, navigating fast-paced health-and-social-care interfaces, and supporting staff learning and development.
