Are you a resilient and fast-paced Social Worker looking for a rewarding role in a clinical setting? We are currently seeking a dedicated Social Worker to join the busy and highly collaborative Hospital Discharge Team based in Rochdale.
In this critical role, you will work at the interface of health and social care. Your expertise will be vital in ensuring that patients experience safe, timely, and effective discharges from the hospital, helping to maximize their independence while alleviating pressure on acute hospital beds.
The Role:
As a Hospital Discharge Social Worker, you will operate within a fast-paced, multi-disciplinary environment. Your key responsibilities will include:
- Facilitating safe and timely discharges using the Discharge to Assess (D2A) model.
- Conducting urgent Care Act assessments and completing comprehensive risk assessments for patients ready to leave the acute ward.
- Completing Mental Capacity Act (MCA) assessments and Best Interests decisions.
- Working seamlessly within a Multi-Disciplinary Team (MDT), collaborating daily with doctors, nurses, occupational therapists, and families.
- Arranging appropriate community care packages, reablement services, or transitional placements to support patients' recovery post-discharge.
- Advocating for patients and families during what can be a highly stressful transitional period.
What We Are Looking For:
- Qualifications: A recognized Social Work degree (BA/BSc/MA or equivalent).
- Registration: Active registration with Social Work England (SWE).
- Experience: Proven post-qualifying experience in Adult Social Care, with specific experience in a hospital setting or hospital discharge team highly preferred.
- Skills: Exceptional crisis management, strong knowledge of the Care Act 2014 and MCA 2005, and the ability to make rapid, safe decisions under pressure.
- Resilience: A confident communicator who can advocate for social care values within a medical model.
