What You'll Be Doing
As a key member of the Hospital Discharge Team, you will focus on high-quality, efficient assessments to reduce delayed discharges. You will:
- Carry out robust statutory assessments within a fast-moving hospital environment.
- Design, implement, and monitor creative care packages that support the "Home First" ethos.
- Navigate complex legal and safeguarding frameworks, including Adult Support and Protection (ASP) and Adults with Incapacity.
- Facilitate multidisciplinary discharge planning meetings to ensure all aspects of an individual's care are coordinated.
What We Need From You (The Requirements)
Because hospital discharge requires a strong grasp of statutory processes, we are looking for candidates who can step in with confidence.
- Professional Registration: Must be a qualified Social Worker registered with the SSSC (Scottish Social Services Council).
- Relevant Experience: Background working within adult social care, a hospital setting, or a fast-paced community assessment team.
- Discharge & Assessment Expertise: Confident knowledge of sectioning, care procurement, and risk management in acute or transitional environments.
- Strong Communication: Excellent negotiation and interpersonal skills to manage expectations between medical staff, families, and service users.
