Are you a proactive and dedicated Social Worker with a passion for preventative care? We are looking for an experienced Social Worker to join the fast-paced and highly supportive Adult Early Support Team in Manchester.
This role is focused on the "front door" of adult social care. You will play a crucial role in providing early, short-term interventions that maximize independence, prevent crisis, and delay the need for long-term care.
The Role:
As a key member of the Early Support Team, your day-to-day responsibilities will include:
- Acting as the first point of contact for adults in need of care and support.
- Conducting rapid, strength-based assessments to identify immediate needs and risks.
- Designing and implementing short-term support plans to promote independence and wellbeing.
- Utilizing the Care Act 2014 and Mental Capacity Act 2005 to inform practice.
- Signposting and connecting individuals to appropriate community resources, voluntary sectors, and health services.
- Working collaboratively with multi-agency partners, including occupational therapists, housing teams, and medical professionals.
What We Are Looking For:
- Qualifications: A recognized Social Work degree (BA/BSc/MA or equivalent).
- Registration: Active registration with Social Work England (SWE).
- Experience: Previous experience working within Adult Social Care, ideally in a front door, intake, or early intervention setting.
- Skills: Strong crisis management abilities, excellent communication skills, and a solid understanding of strength-based practice.
- Mobility: A full UK driving license and access to a vehicle is highly desirable.
