Swansea Council is seeking a dedicated Qualified Social Worker to join their 16+ Children's Service / Leaving Care Team. The Impact You Will Make
In the 16+ team, your practice will focus heavily on empowering young people, building resilience, and facilitating smooth transitions out of the care system:
Pathway Planning: Co-produce and review comprehensive Pathway Plans with young people aged 16-21 (and up to 25 if in education), outlining their housing, education, employment, and personal goals.
Transition & Independence Support: Guide care leavers through practical milestones, from securing safe accommodation and managing budgets to accessing higher education or training opportunities.
Statutory Case Management: Fulfill statutory obligations for looked-after young people and care leavers, conducting regular visits and multi-agency reviews.
Holistic Advocacy: Act as a stable, trusted professional for young people, helping them navigate complex systems while keeping their voices and aspirations at the absolute center of all care plans.
Mandatory Candidate Criteria
To meet Welsh regulatory standards and the specialized demands of this adolescent-focused service, you must fulfill the following:
Professional Registration: You MUST hold an active registration with Social Care Wales to practice.
Relevant Experience: Proven post-qualifying experience within a statutory Children's Services team, ideally with direct experience working with adolescents, looked-after children, or care leavers.
Driving License: A full UK driving license with access to a vehicle for regular community and placement visits.
