Are you an experienced Adult Social Worker looking for a rewarding community-based locum role with excellent hybrid flexibility?
We are currently seeking a skilled Adult Community Social Worker to join an established team based in Birkenhead. Offering a competitive pay rate of £36.08 per hour, this position provides a great work-life balance with a 36-hour working week and a predictable hybrid schedule (2 days remote working, 3 days office/community-based).
Key Responsibilities:
- Statutory Assessments: Conduct comprehensive Care Act (2014) assessments, eligibility determinations, and strengths-based outcome assessments for adults with complex social care needs.
- Support Planning & Reviews: Develop person-centred support plans and undertake timely, robust annual and complex care reviews.
- Safeguarding Adults: Lead on section 42 safeguarding enquiries, manage risk assessments, and implement protection plans to promote individual safety and wellbeing.
- Community Engagement: Work collaboratively with multidisciplinary partners, health colleagues, carers, and community organizations.
What We Are Looking For:
- Professional Registration: Active registration with Social Work England (SWE).
- Core Experience: Proven post-qualifying experience in Adult Community Teams, carrying out Care Act assessments, support planning, reviews, and adult safeguarding investigations.
- Key Skills: Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and strengths-based practice models, with excellent report-writing abilities.
