Are you an experienced, confident Adult Social Worker looking for a lucrative locum opportunity within acute hospital settings?
Liverpool City Council is currently seeking skilled Hospital Social Workers to join their dynamic Transfer of Care and Hospital Discharge teams. Offering competitive pay rates of £42.00 to £46.00 per hour, this role is ideal for practitioners who thrive in fast-paced health environments and are passionate about facilitating safe, timely discharges.
Key Responsibilities:
- Discharge Planning: Manage complex cases across Discharge to Assess (D2A) pathways (Pathways 1-3) to ensure safe, sustainable hospital discharges.
- Statutory Assessments: Complete timely Care Act (2014) assessments, Mental Capacity Assessments (MCA), and Continuing Healthcare (CHC) screenings.
- Multi-Agency Working: Collaborate seamlessly with NHS multidisciplinary teams (MDTs), occupational therapists, discharge hubs, and community care providers.
- Safeguarding: Identify, assess, and manage complex adult safeguarding enquiries within acute healthcare settings.
What We Are Looking For:
- Professional Registration: Active registration with Social Work England (SWE).
- Experience: Proven post-qualifying experience within a Hospital Discharge team, Intermediate Care, or Acute Adult Social Care environment.
- Legal Knowledge: Deep working knowledge of the Care Act 2014, Mental Capacity Act 2005, and Deprivation of Liberty Safeguards (DoLS).
- Skills: Excellent risk management skills, resilience under pressure, and strong communication skills with health partners.
