Are you an experienced Adult Social Worker skilled in acute hospital discharges, intermediate care planning, and multi-agency discharge coordination?
Salford City Council is urgently recruiting two Experienced Social Workers to join their fast-paced Hospital Services & Intermediate Care Team based on site at Salford Royal Hospital. Offering £35.00 per hour, these roles are crucial to driving safe, timely discharges and supporting rehabilitation and recovery outcomes across Salford.
About the Role & Service Area:
Based directly within the hospital environment, you will work collaboratively alongside multidisciplinary health colleagues, ward staff, occupational therapists, and community reablement teams. Your primary focus will be delivering rapid, person-centred social work assessments, managing complex discharge pathways, and facilitating intermediate care packages to prevent unnecessary hospital stays and support safe community transitions.
Key Responsibilities:
- Hospital & Intermediate Care Assessments: Undertake statutory Care Act 2014 assessments and discharge evaluations for patients across acute wards and intermediate care settings.
- Multidisciplinary Discharge Planning: Work alongside NHS consultants, nursing staff, and therapy teams to establish safe, timely discharge and reablement plans.
- Safeguarding & MCA: Complete Mental Capacity Act 2005 assessments, Best Interest decisions, and manage adult safeguarding enquiries within an acute context.
- Streamlined Working: Work a structured 08:00 - 16:00, Monday to Friday schedule to align directly with acute discharge workflows.
What We Are Looking For:
- Professional Registration: Active registration with Social Work England (SWE).
- Experience: Proven post-qualifying experience in an acute hospital, intermediate care, reablement, or rapid-response Adult Social Care setting.
- Knowledge: Expert understanding of the Care Act 2014, Mental Capacity Act 2005, and Hospital Discharge guidance.
- On-Site Availability: Ability to work on-site at Salford Royal Hospital (100% office/hospital-based).
