About the Role
We are looking for a passionate and dedicated Social Worker to join a supportive and fast-paced Learning Disabilities Team in Leeds. In this highly rewarding role, you will work closely with adults with learning disabilities, their families, and carers to promote independence, wellbeing, and social inclusion.
You will play a vital role in ensuring that vulnerable adults receive the right support at the right time, advocating for their needs and empowering them to live fulfilling lives within their community.
Key Responsibilities
- Conduct comprehensive needs assessments and reviews in line with the Care Act 2014.
- Undertake Mental Capacity Act (MCA) assessments and facilitate Best Interest decisions.
- Manage a varied caseload of adults with mild to profound learning disabilities, autism, and complex needs.
- Lead on Section 42 Safeguarding enquiries to protect vulnerable adults from abuse or neglect.
- Develop, implement, and review person-centered care plans that maximize independence and choice.
- Work collaboratively within a multidisciplinary team, liaising with health professionals, housing providers, and community services.
Essential Requirements
- Professional Qualification: A recognized degree or diploma in Social Work.
- Registration: Active registration with Social Work England.
- Experience: Demonstrable post-qualifying experience working within Adult Social Care, specifically with adults with learning disabilities or autism.
- Knowledge: Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.
- Travel: A full UK driving license and access to a vehicle are highly desirable to travel across the Leeds area.
