About the Role:
Are you a passionate and dedicated Adult Social Worker looking for a rewarding contract in the North East? Newcastle Council is currently seeking an experienced Social Worker to join their dynamic Older Persons Team.
In this role, you will work directly with older adults and their families to promote independence, manage risks, and deliver personalized care packages. You will be part of a supportive, multi-disciplinary team environment committed to enhancing the quality of life for the elderly community.
Key Responsibilities:
Managing a varied caseload of older adults requiring social care support and intervention.
Conducting comprehensive care act assessments, support planning, and timely reviews.
Identifying and managing safeguarding risks in line with local policies and the Care Act 2014.
Collaborating with health professionals, community networks, and families to facilitate safe hospital discharges and long-term care plans.
What We Are Looking For:
Social Work England (SWE) Registration is essential.
Proven post-qualification experience within an Adults' Statutory Service (ideally an Older Persons or Locality team).
Strong assessment, report-writing, and communication skills.
Ability to work effectively in a fast-paced environment and manage complex caseloads.
