The Role Are you a highly experienced Social Worker seeking an exceptional hourly rate without compromising your work-life balance?
Liverpool Council is looking for a dedicated and skilled practitioner to join their Adult Locality Team on a part-time basis. This community-based role focuses on assessing and supporting older adults and individuals with physical disabilities to live as safely and independently as possible within their own homes.

🔍 Key Responsibilities:
- Community Assessments: Carry out complex strengths-based assessments and reviews in the community in accordance with the Care Act 2014.
- Safeguarding: Respond to and manage safeguarding concerns, undertaking Section 42 enquiries when necessary.
- Capacity & Consent: Apply expert knowledge of the Mental Capacity Act (MCA) 2005 to conduct mental capacity assessments and make complex Best Interest decisions.
- Care Planning: Develop, implement, and review personalized care and support plans that maximize independence and utilize community resources.
- Multi-Agency Working: Collaborate closely with health colleagues, primary care networks, housing departments, and voluntary sectors to ensure holistic care.
📋 What You Need to Succeed:
- Qualifications: A recognized Social Work qualification (Degree/DipSW/CQSW).
- Registration: Current and active registration with Social Work England (SWE).
- Experience: Substantial post-qualifying experience working within an Adult Community, Locality, or Hospital Discharge team.
- Knowledge: Comprehensive understanding of statutory adult social care legislation.
- Mobility: A full UK driving license and access to a vehicle for community visits across Liverpool.
