Tameside Council's Adult Social Care division is seeking a passionate and legally literate Transitions Mental Health Social Worker to join the dedicated service at Wilshaw House. Summary of Job
The position delivers a comprehensive social care service aimed at young individuals moving away from children's frameworks and into adult mental health services.
The designated caseload primarily supports young people experiencing moderate to severe mental health problems or those with a dual diagnosis of autism.
The practitioner acts as a specialist transition link, requiring extensive legal knowledge of both adult care frameworks and children's education structures.
Duties & Responsibilities
Assesses the complex needs of young individuals, placing a strong operational emphasis on early intervention and preventative care.
Coordinates the formal transition of children approaching their 18th birthday securely into Adult Mental Health provisions.
Integrates specialist knowledge of Special Educational Needs and Disabilities (SEND) and Education, Health and Care Plans (EHCPs) into care paths.
Co-produces support plans post-Care Act assessment, identifying unmet needs and applying for appropriate independent sector funding.
Navigates complex legal frameworks to complete court applications, Copdol packages, and detailed risk assessments.
Establishes trust and robust lines of communication with vulnerable young people and families who may resist professional intervention.
Networks extensively with education, housing, health, and children's services to deliver cohesive, multi-agency care coordination.
Formulates clear funding panel presentations while remaining strictly mindful of the cost implications of care planning.
Candidate Requirements
Qualifications: Professional Social Work Qualification (Degree or DipSW), active Social Work England registration, completed ASYE, and a full UK driving licence with a car available for daily community duties.
Experience: Minimum of 2 years post-qualification experience, with specific, substantial experience in mental health and transitions.
Employment Package
Salary: £39,862 - £45,091 per annum.
Hours & Patterns: Full-time (36 hours per week, Monday to Friday) with flexibility for occasional Saturdays to deliver planned community interventions.
Benefits: 1 agreed hybrid working-from-home day per week, up to 31 days annual leave based on service history, and access to local retail and fitness discounts.
